DVD slideshow order form

Information

You can use this form to order your DVD slideshow or for ordering extra copies of a slideshow. If a slideshow was included in your package then no fees are applicable unless you want to use your own music.

If your package didn't include a slideshow you can still order one but there is a fee of £165 for the first copy and £25 for extra copies.

How many copies are you ordering?
Copies of an existing slideshow are £25 each. If a slideshow wasn't included as part of your package then the fee is £165 for the first copy.

Don't include the following images
List of any images you DON'T want included in your slideshow. Use the reference numbers from your web gallery. The slideshow is a re-edit of the web gallery anyway - I don't include all the images as there would be too much repetition.

Format required
I can author the slideshows in a variety of formats but it may be best to order widescreen hi-definition so your DVD is future proof

I use royalty free classical music for the slideshows. If you would like to use your own music then there is a charge of £55 for a licence. It is illegal otherwise!

Please use these tracks
Enter the names of the tracks and the artist, in the order you'd like them used. Please send them to me on a disc ASAP in mp3 format. I need at least 30mins of music and you will need to pay for the licence.

Notes

Please make sure you order enough copies. DVD slideshows make a great thank-you present for parents. However I only keep the files associated with slideshow production for 6 months after I send out the copies, as they take up an enormous amount of disc space. If you decide you want another copy after this time you will have to pay for the disc to be re-authored.
